## The Medical Examination Process

## What the Medical Officers Actually Look At
The assessment focuses on the likely cost of treating your condition in Australia, compared against the Significant Cost Threshold (SCT). Rheumatoid arthritis that is well-controlled on standard DMARDs — methotrexate, hydroxychloroquine, sulfasalazine — generally does not trigger the threshold. The cost profile changes significantly if you are on biologic medications such as adalimumab or etanercept, which can cost $20,000–$50,000 AUD per year before the PBS subsidy applies.
This is the crux of the matter: as a permanent resident, you gain access to Australia's Pharmaceutical Benefits Scheme, which reduces the cost of biologics dramatically.
